Despite human error (which includes violation) continuing to be the dominant (and sometimes only) factor in the majority of accidents, many investigators are unable to identify in layman's terms how it defeats an otherwise effective management system. Some of these, despite having evidence to the contrary, will forcefully side step human error issues in favour of alleged regulatory breaches or management failures as the cause of the accident in question.
